Born and raised in Minnesota, Nicole Tyson is a dedicated Community Health Navigator with a passion for helping individuals and families access the resources, services, and support they need to thrive.

Nicole earned her Community Health Worker (CHW) and also holds certifications as a Health Unit Coordinator (HUC) and Certified Nursing Assistant (CNA). With over 15 years of experience across healthcare, case management, and human services, Nicole brings a wealth of knowledge and experience in supporting clients, coordinating services, identifying resources, and helping individuals navigate complex healthcare and social service systems.

Known for her compassionate, caring, and resourceful approach, Nicole is committed to meeting people where they are and helping remove barriers that may prevent them from receiving the care and support they need. She works collaboratively with clients, families, healthcare providers, community organizations, and other professionals to build strong support networks and address the whole person.

Nicole’s passion for community health is rooted in her belief that everyone deserves to feel heard, supported, and connected to the resources that can improve their health and overall well-being.
